Summary   Chatham Hospitalists protects the privacy of your health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA). This summary describes how we may use and disclose protected health information (PHI), your rights regarding PHI, and how to file a complaint.

Grievance Procedure   If you believe you have been discriminated against, you may file a grievance with our Section 504 Coordinator. You may also file a civil rights complaint with the U.S. Department of Health & Human Services, Office for Civil Rights.

No Surprises Act   You have the right to receive a Good Faith Estimate explaining expected medical charges. You are protected from surprise billing for emergency services and certain hospital‑based care.
