This Terms of Use agreement governs your access to and use of the Chatham Hospitalists website and any related online services (the Site). By using the Site you agree to these terms. If you do not agree, do not use the Site. These Terms of Use are separate from our HIPAA Notice of Privacy Practices and Website Privacy Statement; those documents govern protected health information and website data practices.

Permitted use   The Site is provided for general informational purposes about Chatham Hospitalists’ services, policies, and contact information. You may view, download, and print content from the Site for your personal, noncommercial use only.

Prohibited use   You must not:

• Use the Site for any unlawful purpose or in a way that violates these terms.
• Attempt to access restricted areas of the Site or interfere with Site operation.
• Reproduce, distribute, modify, publish, or create derivative works from Site content for commercial purposes without our written permission.

Do not submit protected health information (PHI) through unsecured Site forms; PHI submitted through secure clinical channels is governed by our HIPAA Notice of Privacy Practices.

Ownership   All Site content, including text, graphics, logos, images, and software, is owned or licensed by Chatham Hospitalists and is protected by copyright, trademark, and other intellectual property laws.

Trademarks   All trademarks, service marks, and trade names displayed on the Site are the property of their respective owners. You may not use any trademark displayed on the Site without the owner’s prior written consent.

Limited license   Chatham Hospitalists grants you a limited, revocable license to access and use the Site in accordance with these Terms of Use. Any other use requires our prior written consent.

No medical advice   Information on the Site is provided for general informational purposes only and is not medical advice.
